Agency Overview, Public Notice, Your Rights Under Title VI, Title VI complaint Procedure and Complaint Form
The Livingston County Nursing Home District d/b/a Morningside Center is pleased to be able to provide transportation services to the residents who reside at Morningside Center Nursing Home and Assisted Living Apartments. Transportation services include taking residents to local doctor’s appointments, on activity outings, and for personal shopping. Transportation services are provided by Morningside Center at no charge to the residents. Appointments are scheduled based on the availability of staff and vehicles. Families are encouraged to accompany the resides to appointments.
It is the intent of The Board of Directors of the Livingston County Nursing Home District d/b/a Morningside Center Nursing Home and Assisted Living Apartments to comply with the requirements of Title VI of the Civil Rights Act of 1964 and all related nondiscrimination statutes, regulations and laws.
The Board of Directors has adopted and follows the Title VI of the Civil Rights Act of 1964 (42U.S.C.200d), related statutes and regulations provide that no person shall on the ground of race, color, national origin, sex, or disabilities be excluded from participation in, be denied the benefits of, or be subjected to discrimination under any program or activity receiving Federal Funds. One of Morningside Center’s lift vans was purchased in 2004 through a federal grant program.
How to file a Complaint
You may file a signed, written complaint 180 days of the date of the alleged discrimination.
The complaint should include:
- Your name, address and contact number. If you are filing on behalf of someone else, also include their name, address, contact number and your relation to that person.
- The name and address of the agency, institution or department you believe discriminated against you.
- How, why and when you believe you were discriminated against. Include as much specific, detailed information as possible about the alleged acts of discrimination, and any other relevant information.
- The names of any persons, if known, who Morningside Center can contact to clarify your allegations.
Your complaint must be signed and dated.
